How it works

What Is Permanent Supportive Housing?

YWCA Dayton’s permanent supportive housing program pairs affordable housing with voluntary support services, in order to help women who have been chronically homeless get housing and stay housed.

Who do we serve?

YWCA Dayton’s permanent supportive housing program is based in Montgomery County and serves single women, experiencing homelessness, who have a documented disability.

Permanent supportive housing is a cost-effective solution, shown to lower public costs associated with the use of crisis services such as shelters, hospitals, jails and prisons. Learn more.

Services

YWCA Dayton Permanent Supportive Housing units feature:

  • Subsidized rent

  • Fully-renovated private studio and 1-bedroom apartments with
    private kitchenette and bath

  • Common lounge and learning areas

  • Onsite food pantry

  • Onsite case management & life skills support

  • 24/7 security

Case management for residents in Permanent Supportive Housing is voluntary and a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy. Each case management plan is tailored specifically to the needs of each individual and includes:

  • Assessing residents’ physical and mental wellness needs, preferences and abilities to develop care plans

  • Assisting residents with maintaining housing stability

  • Working with residents, family and friend support networks, and health care professionals to put care plans in place

  • Listening to residents’ concerns and providing intervention

  • Evaluating residents’ progress and making adjustments to improve outcomes

  • Advocating for residents’ health and making sure they receive the right care for their situation

All new tenants living at our permanent supportive housing are referred to our program through the Montgomery County Coordinated Entry System.
